Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts systematic, dynamic assessments collecting physiological, psychological, sociocultural, spiritual, economic, and lifestyle data.
- Identifies problems based on clinical judgment about the patient's health needs.
- Plans and establishes measurable goals in collaboration with the patient and family.
- Implements nursing care according to the care plan to ensure continuity during hospitalization and discharge.
- Educates patients and families about care and treatment.
- Documents patient care in the healthcare record.
- Continuously evaluates the patient’s status and the effectiveness of nursing care.
- Coordinates care with patients, families, and other healthcare team members.

Experience: None required
Education: Graduation from an accredited School of Nursing; Preferred: Bachelors of Science in Nursing
License & Certification:
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) - American Heart Association
