Job Responsibilities
- Collect pertinent data and information related to the healthcare consumer's health or situation
- Analyze assessment data to determine actual and potential diagnoses or issues
- Identify expected outcomes for a plan individualized to the healthcare consumer
- Develop and modify a plan of care that prescribes strategies and alternatives to attain expected outcomes
- Implement the identified plan of care
- Coordinate care delivery and transitions in care
- Employ strategies to promote health and a safe environment
- Evaluate progress toward attainment of outcomes

- Current BCLS certification or must obtain within one month of hire
