Job Responsibilities

Provide excellent care to patients along with acute care GIP partners.
Work with patients, their families, and other healthcare professionals during the final stages of patients' lives.
Provide care in private homes, residential care facilities, nursing centers, and other hospice care environments.
Supervise licensed vocational/practical nurses and nursing aides.
Communicate compassionately with patients and families.
Observe patients keenly and maintain high ethical standards.
Alert doctors and others about changes in patients' conditions.

Requirements

Experience: Minimum of one year of clinical experience is strongly preferred.
