Job Responsibilities
- Perform visits and complete coordination of client care.
- Act as the point of contact for all disciplines involved with providing care to patients.
- Oversee the frequency of visits for the patient care episode.
- Consult with the physician and the office regarding patient care details.
- Perform accurate OASIS collection.
- Ensure that the medication profile remains current.
- Ensure that lab values have been reported to the physician timely.
- Attend weekly case conferences and monthly case manager meetings.
