Perform home visits and coordinate client care as the point of contact for all disciplines involved.
Oversee the frequency of visits for the patient care episode.
Consult with the physician and office regarding patient care details as needed.
Perform accurate OASIS collection.
Ensure the medication profile is current.
Ensure lab values are reported to the physician in a timely manner.
Attend weekly case conferences and monthly case manager meetings.

Experience: Minimum of one year of clinical experience preferred.
