Job Responsibilities
- Maintain and enhance knowledge of the cardiac surgery patient through educational opportunities
- Meet departmental/hospital expectations of growth goals set forth by the performance appraisal document
- Strengthen the cardiac surgery service through participation in departmental and hospital activities
- Model and uphold a customer service focus to all cardiac surgery patients and families
- Ensure confidentiality of patient information is maintained
- Collaborate with healthcare team to ensure customer service reflects excellence consistent with Hospital policy
- Provide thorough discharge instructions and ensure medication reconciliation
- Ensure that discharge summary is accurate and complete at the time of discharge
- Establish interventions to decrease the need for emergency services or re-hospitalization
- Identify and analyze trends in customer service/satisfaction
- Provide leadership in resolving any negative trends in satisfaction, quality outcomes, and practice excellence
- Collect clinical data for Surgeon and practice results that includes volume, risk factor assessment, morbidity, and mortality
- Interact with the entire healthcare team to ensure seamless delivery of patient care
- Serve as the direct link between the Attending Cardiac Surgeon and the Referring Physician
- Provide information and support to interdisciplinary team members within the hospital
- Participate in daily rounds and report concerns regarding patient care to the Attending Surgeon
- Communicate complex medical conditions with healthcare providers and insurance companies for procedure precertification
- Orchestrate the Attending Surgeon schedule both in the operating room and the office

Experience: 1-2 years caring for cardiac surgery patients preferred
Education: Associates or Bachelor's Degree in Nursing required

- Licensed as a Registered Nurse from the State of Connecticut
