Job Responsibilities
- Facilitating and coordinating patient care during all phases of transplantation.
- Identifying candidates upon transplant referral and coordinating the evaluation process.
- Assessing and triaging organ specific patient calls for appropriate resolution of issues.
- Facilitating transplant clinic activity.
- Ensuring proper documentation of all patient care issues through the communication center.
- Serving as the voice of the department through frontline, telephonic communication.
- Communicating with candidates, referring physicians, and other health care providers regarding patient status.
- Providing telephonic patient education in transplant and organ donation.
- Advocating on patient/family's behalf for clinical and ethical concerns.
- Viewing educational deficits of candidates/families and providing necessary education.
- Collecting and reviewing data pertinent to recipients' health and organ function.
- Validating patient's insurance authorization for transplant referral process.
- Participating in department initiatives for quality and performance improvement.
Required Skills
Core Skills
- Registered Nurse (RN) License
- Basic Life Support (BLS) Certification
- Strong assessment and organizational skills
- Ability to communicate effectively
- Proficiency in English
Preferred Skills
- Experience in healthcare nursing
- Knowledge of Microsoft products
- Skills in managing complex patient care issues
Requirements
Experience: One year nursing healthcare experience
Education: Bachelor's degree or higher from an accredited School of Nursing
License & Certification:
- RN - Registered Nurse - Texas State Licensure
- BLS - Basic Life Support (AHA)
