Job Title: RN Case Manager, Home Health
Position: Management of patients including direct care and coordination of the interdisciplinary team.
Job Type: Full Time
Pay: $68.42 to $91.00 / hour
Shift: Days, Monday-Friday / Rotating Weekends

Job Responsibilities
- Manage overall care of patients including provision of direct care.
- Plan, organize, and direct all patient care services for assigned caseload.
- Ensure that optimal quality care is maintained according to standards and regulations.
- Coordinate interdisciplinary team for implementation of patient care plan.
- Manage visit utilization and control expenses.
- Assist in agency-wide orientation and competency evaluations.
- Consult with clinical staff related to specific expertise.
Required Skills
Core Skills
- Ability to function independently and as part of an interdisciplinary team
- Demonstrated clinical assessment skills
- Ability to communicate effectively with patients and significant others
- Understanding of palliative care principles
Preferred Skills
- Knowledge of pathophysiology and treatment protocols for common home care diagnoses
- Ability to recognize the special needs of Hospice patients
- Knowledge of safety and infection control principles
- Effective written and verbal communication skills
Requirements
Experience: 2 years recent relevant experience
Education: Graduate of an accredited School of Nursing
License & Certification:
- RN-Registered Nurse of California
- BLS-Basic Life Support Healthcare Provider
- Valid Drivers License Class C
- Automobile Insurance
Schedule:
- Full Time, Rotating Weekends
