Job Responsibilities
- Provide program orientation to patients, families, and caregivers including coverage of the case manager's role and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to care planning.
- Ensure the implementation of the patient's treatment plan, supporting their strengths and preferences.
- Document findings of Discharge Planning Evaluation (DPE) and psychosocial assessments.
- Communicate social, financial, or discharge needs and preferences of patients and families.
- Promote consistent, positive patient interactions to enhance patient service.
Required Skills
Core Skills
- Strong clinical nursing skills
- Experience in case management
- Excellent communication and interpersonal skills
Preferred Skills
- Bachelor's Degree in Nursing (BSN)
- Familiarity with Medicare and insurance benefits
- Ability to conduct Family Conferences
Requirements
Experience: Minimum of two years recent acute care clinical nursing experience and two years recent acute care case management experience required.
Education: Associate Degree in Nursing required; BSN preferred.
License & Certification:
- Registered Nurse with a current California license
