Job Responsibilities
- Assess eligibility for Empath Health Network of Care.
- Advocate for optimal access to the right care at the right time in any care setting.
- Complete the admission process based on care program specifics, guidelines, and patient-specific needs.
- Engage in mission support while exhibiting passion for the services offered.
- Portray excellence in customer service and relationship-building internally and externally.

Requirements
Experience: Minimum of six months to one year related experience and/or training.

- Monday-Thursday 8am-7pm
