Accountable for the provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients.
Delegate appropriate tasks to direct patient care staff including RNs, LVN/LPNs, and Patient Care Technicians.
Perform ongoing, systematic collection and analysis of patient data pre, during, and post hemodialysis treatment, and document in the patient medical record.
Make adjustments to the treatment plan as indicated and notify Team Leader, Charge Nurse, Supervisor, or Physician as needed.
Assess and document patient/family’s basic learning needs for ongoing education.
Direct and provide daily safe and effective hemodialysis therapy delivery to assigned patients.
Administer medications as prescribed and document medical justification and effectiveness.
Initiate or assist with emergency response measures.
Serve as a resource, leader, coach, mentor, and role model for new and incumbent employees.
Ensure correct laboratory collection, processing, and shipping procedures are performed.
Identify expected outcomes and update nursing assessment and plan of care for assigned patients.
Ensure patient awareness related to transplant and treatment modality options.
Participate in education and quality improvement projects at the facility and area level.
May serve as a Preceptor to new employees.
Complete CAP requirements to maintain or advance.

Experience: Minimum of 2 years of Nephrology Nursing experience

Education: Graduate of an accredited School of Nursing
