Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial, re-assessment, and focused assessment based on patient condition, setting, and population.
Identifies problems through clinical judgment about patients' responses to health conditions or needs.
Plans and formulates measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Implements nursing care according to the care plan, ensuring continuity of care.
Ensures patient safety.
Administers medication and provides ordered treatments.
Educates patients and families.
Documents care in healthcare records.
Evaluates continuous patient status and effectiveness of nursing care.
Coordinates care with patients, families, and healthcare team members.
