Job Responsibilities
- Act as the leading provider of internal medicine services at the Skilled Nursing Facility
- Collaborate with the facility’s interdisciplinary team and physicians
- Serve as a day-to-day clinical leader overseeing the care of all residents
- Work with the entire patient population, both short and long-term residents
- Oversee day-to-day care with assistance from the nursing staff and guidance from medical directors
- Evaluate new-admission visits
- Coordinate discharge visits
- Conduct monthly stability visits for long-term residents
- Evaluate patients exhibiting changes in condition
- Provide Advanced Care Planning
- Perform comprehensive health assessments, differential diagnosis, and prescribe treatments
- Manage acute and chronic illness and disease
- Prescribe necessary medications and treatments
- Conduct comprehensive physical examinations of patients
- Develop and implement patient management policies and procedures
- Facilitate referrals to other healthcare professionals and medical facilities
- Communicate with attending physicians and/or specialists regarding patient care
Required Skills
Core Skills
- Advanced Practice Licensure (APN) with a Florida license
- Active license and DEA number
- Ability to build rapport with patients and their loved ones
- Ability to thrive in a fast-paced environment
- Excellent written and verbal communication skills
- Strong leadership experience with emphasis on teamwork in a healthcare setting
- Master’s degree

- Previous experience in nursing or other medical fields
