Position: Senior Community Care Team Member

Job Type: Full Time

Pay: $89,900 - $160,600 annually

Shift: No on-call, no weekends, and no holidays required

Job Responsibilities

  • Provide clinical coverage for the primary APC during periods of PTO or short/long term absences
  • Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions
  • Manage members during a short term, sub-acute rehabilitation episode
  • Conduct comprehensive, monthly/routine, and/or acute care visit assessments
  • Effectively manage acute and chronic medical and behavioral conditions
  • Ensure accurate and complete documentation, including ICD 10 conditions
  • Communicate and collaborate with the interdisciplinary care team

Required Skills

Core Skills

  • Active and unrestricted license in the state of residence
  • Active Nurse Practitioner certification through a national board
  • Current, active DEA licensure/prescriptive authority
  • 3+ years of clinical experience in practice

Preferred Skills

  • 5+ years of clinical experience in practice
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills including documentation skills with written and electronic medical records

Requirements

Experience: 3+ years of clinical experience in practice

Education: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program

License & Certification:

  • Active Nurse Practitioner certification
  • Current, active DEA licensure/prescriptive authority
