Job Title: Nurse Practitioner - PRN
Position: Nurse Practitioner - Home Risk Assessments
Job Type: PRN
Pay: $2,000 up to $10,000 per month based on completion of assessments
Shift: Days, Evenings, Nights, Weekends, Flexible Scheduling

Job Responsibilities
- Conduct Adult/Geriatric assessments including medical history, diagnosis and treatment.
- Provide health education and physician referrals.
- Make case management referrals and provide follow-up care.
- Document assessments clearly according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
- Collaborate with Primary Care Physicians on patient education and follow-up.
- Provide services in various venues including home visits and skilled facility visits.
Required Skills
Core Skills
- Current RN and NP licensure in state of practice including prescription authority or ability to obtain it.
- Board certification by AANP, ANCC or AACCN in a Matrix-approved specialty.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
Preferred Skills
- 1 year experience as a Nurse Practitioner (new grads encouraged to apply).
- Comfort and flexibility with frequent change.
Requirements
Experience: 1 year experience as Nurse Practitioner preferred, new grads welcome.
Education: Master’s Degree or commensurate experience.
License & Certification:
- Current RN and NP licensure in state of practice.
- BLS, ACLS, or CPR certification.
Schedule:
- Flexible scheduling.
- Travel required.
