- Conduct comprehensive Health Risk Assessments for Medicare Advantage patients
- Review patients' medical histories
- Understand patients' social environments
- Enhance care quality by providing significant insights during visits
Required Skills
Core Skills
- Board-certified Nurse Practitioner
- Licensed in Colorado
- At least 1 year of Nurse Practitioner experience
- Home health experience
