Position: Mobile practice provider in a long-term care setting

Job Responsibilities
- Perform on-site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by the completion of a comprehensive assessment form
- Function independently to perform appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
- Order and review diagnostic and therapeutic tests relative to patient's age-specific needs
- Assist with the provision of care in accordance with facility, state, and federal regulations
- Monitor the effectiveness of interventions
- Maintain relationships with all appropriate contacts in facilities
- Educate family members, clients, and prospective accounts regarding services and programs offered by Primary Vascular Care
- Assist with the development and maintenance of physician relations, as assigned
- Complete weekly, monthly, and other reports as assigned in a timely and comprehensive manner

Requirements
Experience: 1 year of NP experience preferred
Education: Bachelor's Degree required; Degree from an accredited Nurse Practitioner or Physician Assistant Program required
License & Certification:
- Current NP License required
- BLS - Basic Life Support through American Heart Association required
- Current Infection Control Certification required
- Board Certified/Board Eligible required
- Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella required
- COVID and Flu Vaccine preferred
