- Coordinate telephonic care for high-risk patients with chronic conditions
- Develop and implement comprehensive care plans
- Conduct telephonic outreach and medication reconciliation
- Ensure continuity of care across the healthcare ecosystem
- Act as an advocate for patients and integrate services across providers
- Manage care transitions for patients discharged from inpatient settings
