Job Responsibilities
- Direct Patient Care Technician's provision of safe and effective delivery of chronic hemodialysis therapy to patients.
- Delegate tasks to direct patient care staff including LVN/LPNs, Patient Care Technicians, and Dialysis Assistants.
- Assess daily patient care needs and develop appropriate patient care assignments.
- Monitor patient care staff for appropriate techniques and adherence to facility procedures.
- Participate in staff training and orientation of new staff as assigned.
- Participate in all required staff meetings.
- Ensure educational needs of patients and families regarding End Stage Renal Disease (ESRD) are met.
- Provide ongoing education to patients regarding their renal disease and dialysis therapy.
- Discuss and record patient education related to diet, fluid, and medication compliance.
- Provide detailed education regarding adequacy measures.
- Ensure transplant and modality awareness is communicated to patients.
- Provide safe and effective delivery of care to patients with ESRD.
- Accurately implement treatment prescriptions.
- Assess patient responses to treatment and adjust the plan as needed.
- Communicate problems or concerns to the Team Leader or physician.
- Initiate Initial and Annual Nursing Assessment according to FMC policies.
- Participate in pre-evaluation, initiation, monitoring, and termination of hemodialysis therapy.
- Take appropriate interventions for changes in patient adequacy status.
- Ensure accurate and complete documentation by Patient Care Technician.
- Review, transcribe, and enter physician lab orders into the Medical Information System.
- Ensure lab requisitions and specimens are prepared and processed correctly.
- Report alert/panic and abnormal lab results to the physician.
- Maintain shift operation in a safe, efficient, and effective manner.
- Collaborate with healthcare team members to interpret and coordinate patient care.
- Administer prescribed medications and document medical justification when necessary.
- Ensure a clean, safe, and sanitary environment in the dialysis facility.
