Job Responsibilities

Work as part of an interdisciplinary team to ensure high quality outcomes for family members struggling with chronic disease management.
Case management and care coordination for previously non-engaged members.
Develop plans of care utilizing available team and community-based resources.
Conduct appropriate assessments resulting in nursing care plans prioritized by the patient and caregivers.
Perform in-home, virtual, or tele-health assessments as directed.
Track nursing care plan outcomes, interventions, and reassess patient needs as appropriate.
Utilize care pathway templates by condition with risk levels and member actions by event type.
Respond real-time to member clinical escalations to triage acute care issues.
Provide care coordination including patient navigation, chronic disease management, and interdisciplinary collaboration.
Engage members in managing their health, medications, treatment, and mental health needs.
Follow evidence-based guidelines to facilitate closure of gaps in care.
Use electronic medical records and clinical management platforms for care coordination.
Participate in team-based rounds for ongoing program design and development.
Identify and shift priorities to effectively manage patient care load.
Lead in defining and executing strategies for clinical practice business value.
Establish business strategy to improve outcomes for family members and develop external market opportunities.
Ensure clinical capabilities form a cohesive offering across business divisions.
Provide subject matter expertise in clinical solutions and highest quality medical care.
Create a productive, collaborative, safe, and inclusive work environment.

Required Skills

Core Skills

Strong clinical assessment and nursing process skills
Analytical and critical thinking abilities
Communication skills including verbal, written, and presentation
Ability to build and manage relationships

Preferred Skills

Experience with Electronic Medical Records (EMRs)
Experience with Microsoft Office products
Previous experience in care management within health plans, hospitals, or hospice

Requirements

Experience: At least 3 to 5 years of relevant clinical experience, preferably 3+ years in care management.

Education: BSN Required; MSN or other healthcare related graduate degrees a plus.

License & Certification:

Current & active RN license in the state of practice
BLS certification
