Job Responsibilities
- Provides direct patient care as defined in California Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care and prepares clinical notes.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
- Uses equipment and supplies effectively and efficiently.
- Provides appropriate pain/symptom management.
- Evaluates patient’s response to treatments/medications.
- Participates in personal, professional growth and development.
- Participates in quality assessment performance improvement teams and activities.
- Performs other duties as assigned by the registered nurse.
Required Skills
Core Skills
- Good verbal and written communication skills
- Organization skills
- Understanding of the philosophy of hospice concept
Preferred Skills
- Community/home health or medical/surgical experience
- CPR certification
Requirements
Experience: Three years nursing experience, preferred community/home health or medical/surgical experience
Education: Graduate of an accredited practical nurse or vocational nursing program
License & Certification:
- Licensed practical nurse or licensed vocational nurse in California
- Current CPR certification
