Job Responsibilities
- Processes patient phone calls/messages and routes them to providers as appropriate.
- Provides test results to patients along with any updates to current plan of care.
- Completes nurse visits with patients to include: assessment, medication reconciliation, vitals and EKG, evaluation and implementation of clinical care plan as instructed by provider.
- Handles prescription refills.
- Coordinates FMLA, Disability, Surgical Clearances and other forms.
- Delivers patient education.
- Performs other clinical duties as assigned.
Required Skills

- Strong collegial relationships
- Professional knowledge of nursing theory and practice
- Cardiology experience preferred
Preferred Skills
- BLS for Healthcare Provider required or must be obtained within 60 days of hire
Requirements
Experience: Cardiology experience preferred, but not required
Education: Current state license as a Practical Nurse (LPN) or Practical Nurse holding a valid Compact State License
