Job Responsibilities
- Provide coordination of care and ensure continuous assessment of each patient's and family's needs.
- Implement the interdisciplinary care plan.
- Perform services in accordance with the physician’s orders and the established plan of care (POC).
- Ensure delivery of care is compliant with federal and state regulations.
Required Skills
Core Skills
- Graduated from an approved school of nursing education
- Licensed as a registered nurse (RN) in the state of practice
- Basic technology skills, including operation of a mobile device
Preferred Skills
- Medicare home health or hospice experience
Requirements
Experience: At least one year experience as a licensed registered nurse (RN)
Education: Graduated from an approved school of nursing education
License & Certification:
- Valid state driver license
- CPR certification for healthcare provider
Schedule:
- 7 days on (5pm-8am) / 7 days off
