Job Responsibilities
- Complete Start of Cares and Resumption of Cares.
- Perform 2nd Service Evaluations, Routine/Recertification Visits, and Discharge Visits as needed.
- Completes weekly schedule and makes scheduled home visits per assignment.
- Schedule and prioritize visits according to client acuity and in accordance with the organization's emergency preparedness plan.
- Develop individualized and holistic plan of care for the patient in collaboration with the patient/family/caregiver during admission visits.
- Perform admission physical and psychosocial assessments to determine appropriate plan of care.
- Implement the plan of care, nursing interventions, and procedures as evidenced by chart review and observation during each visit.
- Update the plan of care on an ongoing basis as goals are met and problems are identified.
- Document improvements in outcomes or applicable variances for discharges.
- Prepare patients appropriately for discharge in coordination with all disciplines involved in care.
- Administer and document medications according to department policies and procedures.
- Complete medication reconciliations every visit and update medication profiles as appropriate.
- Teach medication management to patients and family members, including side effects.
- Coordinate the plan of care for each patient, ensuring patient/family involvement.
- Communicate with all disciplines to ensure quality of care.
- Foster a 'Culture of Safety' through personal ownership and commitment to a safe environment.
- Comply with current hand hygiene and infection control guidelines.
- Participate in opportunities for professional development and enhancement of nursing image.
- Demonstrate use of advanced clinical skills and knowledge in practice.
Required Skills
Core Skills
- Basic Nursing competencies
- Knowledge of medication administration policies and procedures
- Excellent communication skills in English
Preferred Skills
- Bachelor of Science Degree in Nursing preferred
- 1–2 years of medical/surgical nursing experience preferred
Requirements
Experience: 1-2 years of medical/surgical nursing experience preferred
Education: Bachelor of Science Degree in Nursing preferred
License & Certification:
- RN-Registered Nurse - Board of Registration in Nursing (Massachusetts)
Schedule:
- 36 hours per week
- 8 AM - 5:30 PM
- 1 weekend every 4 weeks (Saturday & Sunday) with comp days the week prior and following
- 1 holiday per year
