Job Responsibilities

  • Provide clinical coverage for the primary APC during periods of PTO or short/long term absences.
  • Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions.
  • Manage members during a short term, sub-acute rehabilitation episode.
  • Conduct comprehensive, monthly/routine, and/or acute care visit assessments.
  • Effectively manage acute and chronic medical and behavioral conditions.
  • Ensure accurate and complete documentation, including ICD 10 conditions.
  • Communicate and collaborate with the interdisciplinary care team.
  • Conduct advanced illness and advanced care planning conversations.
  • Provide patients and caregivers with counseling and education.

Required Skills

Core Skills

  • Active and unrestricted license in the state of Florida
  • Certified Nurse Practitioner or Physician Assistant
  • Graduate of an accredited master’s degree in Nursing (for NPs)
  • Current, active DEA licensure/prescriptive authority
  • 1+ years of hands-on post grad experience in a long-term care setting
  • Ability to complete physical job requirements including lifting a 30-pound bag and navigating various dwelling conditions
  • Driver's License and access to reliable transportation

Preferred Skills

  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional skills
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including ability to document medical information

Requirements

Experience: 1+ years of hands-on post grad experience (long-term care setting preferred)

Education: Graduate of an accredited master’s degree in Nursing (MSN) program for NPs; Graduate of an accredited Physician Assistant degree program for PAs

License & Certification:

  • Active and unrestricted license in Florida
  • Certified Nurse Practitioner or Physician Assistant through a national board
  • Current, active DEA licensure/prescriptive authority

Schedule:

  • No on-call, no weekends, and no holidays required
