Job Responsibilities

Provide clinical coverage for the primary APC during periods of PTO or short/long term absences.
Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions.
Manage members during a short term, sub-acute rehabilitation episode.
Conduct comprehensive, monthly/routine, and/or acute care visit assessments.
Effectively manage acute and chronic medical and behavioral conditions.
Ensure accurate and complete documentation, including ICD 10 conditions.
Communicate and collaborate with the interdisciplinary care team.
Conduct advanced illness and advanced care planning conversations.
Provide patients and caregivers with counseling and education.

Required Skills

Core Skills

Active and unrestricted license in the state of Florida
Certified Nurse Practitioner or Physician Assistant
Graduate of an accredited master’s degree in Nursing (for NPs)
Current, active DEA licensure/prescriptive authority
1+ years of hands-on post grad experience in a long-term care setting
Ability to complete physical job requirements including lifting a 30-pound bag and navigating various dwelling conditions
Driver's License and access to reliable transportation

Preferred Skills

Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional skills
Understanding of Geriatrics and Chronic Illness
Understanding of Advanced Illness and end of life discussions
Proficient computer skills including ability to document medical information

Requirements

Experience: 1+ years of hands-on post grad experience (long-term care setting preferred)

Education: Graduate of an accredited master’s degree in Nursing (MSN) program for NPs; Graduate of an accredited Physician Assistant degree program for PAs

License & Certification:

Active and unrestricted license in Florida
Certified Nurse Practitioner or Physician Assistant through a national board
Current, active DEA licensure/prescriptive authority

Schedule:

No on-call, no weekends, and no holidays required
