Job Responsibilities

  • Perform professional nursing duties for outpatient clinical areas.
  • Treat and educate patients, their families, and the public regarding various medical conditions.
  • Record medical histories and symptoms.
  • Perform diagnostic tests and analyze results.
  • Operate medical equipment and administer treatment and medications.
  • Make assessments and create a plan of care.
  • Perform care coordination and case management.
  • Participate actively in the team-based care model.
  • Provide lead direction to other nursing and paraprofessional personnel.
  • Administer medications, immunizations, and perform medication reconciliation.
  • Titrate medications/manage refills per clinical guidelines.
  • Complete patient assessments and triage, recording patient histories, symptoms, and observations.
  • Critically evaluate and use good judgment in unusual or emergent situations.
  • Utilize population health methodologies to improve patient and population outcomes.
  • Organize, supervise, and accept responsibility for the quality of care provided by co-workers.
  • Participate in Performance Improvement/Quality Control activities.
  • Implement patient care standards, infection control standards, and quality assurance criteria.
  • Conduct accurate assessment of patient conditions via face to face or telehealth.
  • Provide patient-centered care based on evidence-based guidelines.
  • Offer clinical support, care coordination and direct case management including patient education.

  • Clinical skills related to vital signs, wound care, catheterization, Point of Care Testing and phlebotomy

Preferred Skills

  • Certification in area of specialty
  • Experience in ambulatory clinics

Requirements

Experience: Six months of full-time recent experience as an RN in a comparable area of specialty; current experience of at least 2 years as an LVN may substitute.

Education: Graduate of an accredited Registered Nursing Program; Bachelor's degree in Nursing or higher preferred.

License & Certification:

  • BLS - Basic Life Support Certification issued by the American Heart Association
  • Valid license to practice as a Registered Nurse in the State of California
  • Existing (or obtain new if needed) NPI number
