Position: Clinical Operations Manager for Home Health

Job Type: Full Time

Pay: $77,200 - $106,200 per year, eligible for a bonus incentive plan

Job Responsibilities

  • Develops, plans, implements, analyzes, and organizes clinical operations for a specific location.
  • Conducts/delegates the assessment and reassessment of patients, including updating care plans and interpreting patient needs.
  • Manages the assignment of caregivers and oversees the delivery of care to all patients served by the location.
  • Receives and reviews case referrals, including determining home health or hospice needs.
  • Ensures patients meet admission criteria and makes the decision to admit patients to service.
  • Instructs and guides clinicians for effective performance and delivery of quality home care services.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, prioritizing, and developing patient Plan of Care (POC).
  • Monitors cases to ensure documentation compliance with regulatory agencies and third-party payer requirements.
  • Coordinates communication between team members, attending physicians, and caregivers for planning outcomes.
  • Works with the Branch Director and Company Finance Department to establish location’s revenue and budget goals.
  • Participates in sales and marketing initiatives.
  • Supervises all clinical employees assigned to a specific location, handling employee corrective action and discipline.
  • Participates in interviewing, hiring, training, and development of direct care clinicians.
  • Evaluates performance of staff relative to job goals and requirements, providing coaching and recommending training programs.
  • Assesses staff education needs and conducts regular staff education as necessary.
  • Analyzes situations and identifies problems, utilizing Performance Improvement principles.
  • Has responsibilities for QA/PI activities and collaborates with Utilization Review staff on data tracking.
  • Competently performs patient care assignments and staff management activities, providing direct patient care in emergencies.
  • Acts as Branch Director in his/her absence.
  • Ensures compliance with Company standards and external regulatory requirements.
  • Maintains relationships with referral/community sources and participates in professional organizations.
  • Performs other related duties as assigned or requested.

Required Skills

Core Skills

  • Registered Nurse with current state license.
  • Ability to assess patient needs and develop care plans.
  • Experience in management within home care or hospice environments.
  • Strong communication and supervisory skills.

Preferred Skills

  • Experience in home health or hospice care.
  • Management experience preferred.
  • Capability of monitoring compliance to regulatory standards.

Requirements

Experience: Two years as a Registered Nurse in a home care, hospice or equivalent environment, with at least one year of management experience preferred.

Education: Graduate of an accredited School of Nursing.

License & Certification:

  • Current state license as a Registered Nurse.
  • Proof of current CPR.
  • Valid driver’s license and auto insurance.
