Provide essential personal care under the direction and supervision of a registered nurse.
Assist patients with personal care to preserve good hygiene and provide range of motion.
Report changes in patient’s condition and needs to Supervisor or RN.
Maintain records of services performed and patient's apparent condition.
Establish and maintain a good working relationship with the patient/family and other caregivers.
Participate as an interdisciplinary team member to provide input for patient care.
Possess knowledge of the Medicare COPs and state regulations to ensure quality patient care.
Participate in on-call coverage to ensure patient care.
Engage in continuing education and in-services.
Maintain confidentiality of patient information and business trade practices.
Report incidents and complaints according to Agency policy.
Maintain a cooperative attitude with staffing, promoting teamwork, effectiveness, and efficiency.

Experience: Preferred - 1 year home health/hospice experience.

Education: Completion of State-approved CNA certification training program.

License & Certification:

Nursing Assistant Certification (Vendor Managed)
National Provider BLS - American Heart Association (to be obtained within 30 days of hire)
