Job Responsibilities
- Coordinate services for hospitalized patients whose care needs range from observation level of care to emergent/urgent higher level of care transitions.
- Facilitate payer authorization and transportation for patients.
- Triage hospitalized patients to ensure appropriate transitions to care settings.
- Use critical thinking skills to assist providers in establishing plan of care recommendations.
- Partner with providers and Physician Advisors/Medical Directors to review complex cases.
- Optimize care coordination with hospital, ER, nursing homes, assisted living facilities, and community resources.
- Facilitate planning for patient/family needs to ensure smooth transitions across the continuum of care.
- Coordinate provider aspects of federal/state organization functions per Conditions of Participation.
- Support the Medical Staff Quality Review process by identifying issues related to quality indicators.
- Create and update Plan of Care (POC) with patient/family-centered goals.
- Maintain knowledge of community resources and payer benefits.
- Assist with applications for funding, guardianship, and charity care programs as needed.
- Evaluate current treatment plans to identify barriers and seek alternatives.
- Coordinate team efforts with support services departments for smooth discharge transitions.
- Be a visible resource on the unit and coordinate interdisciplinary team huddles/communication.
- Communicate patient Healthcare needs based on practice standards.

Requirements
Experience: 2+ years of Acute Experience; 2+ years of Registered Nurse Experience.
Education: Completion of a nursing education program approved by a board of nursing.
License & Certification:
- Registered Nurse (RN) - South Dakota Board of Nursing
