Job Responsibilities
- Promote achievement of optimal clinical and resource outcomes.
- Accountable for designated assignments that are complex and resource intensive.
- Facilitate appropriate lengths of stay (LOS) and reimbursement for hospital admissions.
- Ensure admission and continued stay are medically necessary, communicating clinical information to payers.
- Analyze variances to identify opportunities for improvement.
- Act as key information resource for the interprofessional health care team.
- Promote and maintain compassionate quality of care through collaboration.
- Lead projects or perform specialized responsibilities.
- Act as a role model to team members and exemplify effective communication skills.
- Collaborate with the patient care team to develop clinical pathways and care management programs.
- Serve as a primary information resource for case management staff, payers, physicians, and customers.
- Mentor new case management employees.
- Initiate improvement strategies for employee engagement.
- Lead comprehensive case management activities to assess patients and set discharge plans.
- Resolve complex problems and issues independently.
- Implement initiatives to improve patient and family satisfaction.
- Identify need for case management interventions and implement strategies for care facilitation.
- Actively participate in system-wide projects regarding case management.
- Oversee management of high-risk, high-cost patient populations.
- Communicate medical information required by external review entities.
Required Skills
Core Skills
- Registered Nurse (RN) with state licensure
- Expert knowledge of discharge planning and case management
- Strong assessment and problem-solving skills
- Proficient in Microsoft Office Suite
- Ability to communicate effectively with a customer service focus
Preferred Skills
- Master's degree in nursing
- Case Management Certification (ACM or ANCC)
- Knowledge of community resources and health care financial issues
- Experience in leading case management initiatives
Requirements
Experience: Seven years hospital clinical nursing experience, including five years in case management.
Education: Bachelor's degree or higher in nursing, Master's degree preferred.
License & Certification:
- Registered Nurse - Texas State Licensure
- Temporary State Licensure (if applicable)
- Case Management-related Certification (ACM or ANCC)
